We are seeking a compassionate and organized Care Manager to oversee and coordinate care services for our clients in a home care setting. The Care Manager will serve as the primary point of contact for clients and their families, ensuring personalized, high-quality care that promotes independence and well-being. Additional territories include Port St Lucie and Fort Pierce.

Key Responsibilities (but not limited to):

Access clients’ needs through in-home evaluations and collaborate with families and healthcare providers.

Develop and implement individualized care plans tailored to each client’s preferences and medical requirements

Assist in training and supervising care staff, including caregivers, home health aides and certified nursing assistants

Monitor service delivery to ensure quality standards are met and adjust care plans as needed

Act as a liaison between clients, families, healthcare professionals, third party provider partners and internal teams

Maintain accurate documentation, including care plans, progress notes and compliance records

Ensure compliance with all relevant regulations, licensing, and safety standards

Provide ongoing support and education to clients and their families regarding care options and resources

Qualifications:

Minimum of 5 years of experience in home care, healthcare, or social services

Knowledge of home care regulations, healthcare practices, and community resources

Excellent interpersonal, communication, and organizational skills

Ability to assess client needs and develop appropriate care plans

Leadership skills and experience supervising staff

Compassionate, patient, and dedicated to improving clients’ quality of life

Certifications Required: CPR

Education:

Bachelor’s degree in Nursing, LPN License, Social Work or related field.
