What you will be doing?
Manager of Care Management (RN or MSW) at our Medical Center in Fullerton, CA. This is a full-time position that will lead and manage the Care Management Team of RNs and Social Workers.
Under the direction of the Director, Care Management, the Manager, Care Management is responsible for the overall daily operations and clinical practice of the assigned service areas. This position will oversee Case Managers and Care Management Coordinator support staff to assure the proper case management of our patients through the continuum of care. The Manager, Care Management ensure the Case managers to advocate for and assist the patient in the achievement of optimal health, access to care, and appropriately utilizing resources.

Required qualifications:
Bachelor’s degree in nursing or master's degree in social work.
3 years of Experience in an ambulatory or acute care setting.
Strong project management and operations experience.
Preferred qualifications:
Master's degree in nursing or health administration.
Case Management Certification (CCM or ACM) upon hire
3 years of Management experience in a Healthcare setting.
