Job Description

Position Title: Consultant – Care Management

Location: IU Health North Hospital

Shift: Full-Time, Monday - Friday 8:00 am - 4:30 pm

The Care Management Consultant works closely with all relevant stakeholders to train and educate on various aspects of the Integrated Care Management (ICM) process, aiming to achieve top-quality patient outcomes. This role supports the ICM team in executing appropriate discharge plans, including referrals to home and extended care facilities. The Consultant ensures compliance with CMS guidelines through timely documentation and plays a key role in decreasing readmissions by securing post-discharge primary care appointments. Additionally, the role involves monitoring the outcomes of post-acute vendors and ensuring timely follow-up on patient concerns to support the IU Health Collaborative’s quality metrics.

Key Responsibilities:

Consult with all necessary stakeholders to train and educate on the Integrated Care Management process to optimize patient care quality.

Support the ICM team to facilitate safe and appropriate discharge practices, including referral placement to home and extended care facilities. Ensure documentation complies with CMS guidelines by delivering required information promptly.

Assist in reducing hospital readmissions by securing post-discharge primary care physician appointments for patients.

Monitor and support the outcomes of post-acute care vendors, including timely follow-up of patient concerns.

Track and analyze quality data to ensure alignment with IU Health’s collaborative care standards.

Collaborate with care teams, providers, and community resources to improve patient transitions and care continuity.

Provide ongoing education and support to staff regarding care management best practices.

Qualifications & Requirements:

Bachelor’s Degree in a healthcare-related field or equivalent years of experience.

0-3 years of related experience in care management, case management, or healthcare coordination.

Strong communication skills, with the ability to educate and collaborate effectively with diverse teams.

Advanced working knowledge of Microsoft Excel, Word, and PowerPoint.

Excellent organizational and project management skills.

Strong analytical skills and problem-solving abilities.
