Care Management - Care Coordinator
BASIC PURPOSE OF THE JOB
Under the supervision of the Director for Care Management and Coordination or his/her designee, the Care Navigator acts as a patient advocate and care coordinator to hospital clients. An autonomous role that coordinates, negotiates, procures services and resources for, and manages the care of complex patients to facilitate achievement of quality and cost efficient patient outcomes. Care Navigator will be responsible for coordinating patient’s care within and outside of GRMC.
JOB REQUIREMENTS
Minimum Education
▪ Healthcare professional with degree in health science or equivalent.
▪ Degree in nursing or social work strongly preferred.
Minimum Work Experience
▪ Three (3) to five (5) years experience in care coordination role or clinical practices.
▪ On-island payer knowledge highly desirable.
▪ Experience with manage care or care coordination preferred.
Required Licenses / Certifications
• Case Management certification preferred.
