WHAT YOU'LL DO:
As a Care Management Assistant, you will organize and implement social services to patients post Emergency Department visit and acute care admission. Within this role, you will receive referrals for individuals from at-risk populations from interdisciplinary team member including physicians, Case Managers, staff nurses and other members of the care team, and will be assigned to a team to follow through acute care continuum.
