Care Management and Coordination - Care Navigator
BASIC PURPOSE OF THE JOB
Under the supervision of the Director for Care Management and Coordination or his/her designee, the Care Navigator for cancer center acts as a patient advocate and care coordinator to the hospital’s outpatient cancer center. An autonomous role that coordinates, negotiates, procures services and resources for, and manages the care of complex patients to facilitate achievement of quality and cost-efficient patient outcomes. Care Navigator for Cancer Center will be responsible for guiding patients through the continuum of cancer care within and outside of GRMC. This role provides individualized support, education, and coordination of services to patients and their families, ensuring timely access to quality care, addressing barriers, and enhancing the overall patient experience.
JOB REQUIREMENTS
Minimum Education
- Bachelor of Science degree in Nursing (BSN) required; Master’s preferred.
Minimum Work Experience
- Two (2) to five (5) years of acute care hospital experience as a registered nurse or experience as nurse case manager required.
- Minimum three (3) years of clinical experience in oncology preferred.
- Critical care or emergency department experience highly desirable.
- Experience with manage care, case management, care coordination preferred.
Required Licenses / Certifications
- Current Guam RN license required.
- ACLS certification preferred.
- Certification in Oncology Nursing (OCN) or Oncology Nurse Navigator Certification (ONN-CG) preferred.
- Case Management or Managed Care Nursing Certification preferred.
Required Skills, Knowledge and Abilities
- Demonstrate critical thinking skills and ability to make decisions and determine medical urgency of cases for coordination.
- Demonstrate knowledge and understanding of pathophysiology of disease to be able to determine and anticipate medical needs of the patient especially for off island transfer cases.
- Demonstrate skills in the area of negotiation, communication (verbal and written), conflict resolution, interdisciplinary collaboration, creative problem solving, time management and critical thinking.
- Knowledge of healthcare financing, community and organizational resources, patient care processes, and data analysis,
- Knowledge of care management and care coordination.
- Knowledge of health care community resources to include care management and assistance programs.
- Excellent verbal and written communication skills required.
- Demonstrates flexibility by being able to adapt to changing priorities and regulations.
- Basic computer skills required.
