An exciting opportunity has arisen for a Care Home Dementia Family Carer Support Facilitator to be based within our 60 bedded care homes operated by OPS Residential & Day Services. This role with assist the Team Manager and Service Improvement Manager with the development of the Dementia Strategy in Older Peoples Services, Residential & Day Services. The successful candidate will be responsible of coordinating and facilitating the dementia support groups across the Service. This will involve linking with other providers, statutory and voluntary agencies, including health, to develop positive working relationships compiling information on other relevant and available community resources.

The successful candidate will also be responsible for Contributing to a range of social, recreational and educational activities aimed at providing a stimulating environment through person centered working, fully respecting the wishes and opinions expressed by service users and their carers or advocates and promote independence and social inclusion. This role will also include providing flexible support and enabling people to access activities and achieve their personal outcomes.

The Person

  • Provide care and support which meets the Health and Social Care standards. Must be able to work with other Council services and partner agencies to support the effective planning and delivery of care and support within our service.

  • You should be able to work flexibly and creatively within a dynamic and ambitious team.

  • Must have or be able to work towards the required qualification SVQ 3, as required by the Scottish Social Services Council. Within the timeframes set at point of registration, once qualification achieved it is the person's responsibility to inform Scottish Social Services Council accordingly.

  • Before confirming to your appointment, you will be required to obtain Protection of Vulnerable Groups (PVG) scheme membership through Disclosure Scotland and become a member of the relevant PVG scheme.

  • As this role requires SSSC registration, the successful applicant will have three months to apply for registration.

  • If appointed, you must start your application as soon as possible so you're registered within the required six months. More details are included in the role profile.
