At Uniting, we’re all about people and supporting older Australians to live independently, with dignity and choice. We’re looking for a Care Excellence Lead to bring clinical expertise, leadership and passion to our Home and Community Care teams across Western and Southwestern Sydney.
This is a unique opportunity to step into a strategic leadership role where you’ll shape practice, strengthen capability, and drive high-quality, person-centred care.
What you’ll be doing
• Provide clinical leadership, mentoring and support to frontline teams to ensure safe, high-quality care
• Champion clinical governance, risk management and quality improvement initiatives
• Support the delivery of care that is person-centred, outcomes-focused and aligned to individual goals
• Lead and contribute to clinical audits, compliance, and continuous improvement activities
• Provide expertise in complex clinical care for older people living in the community
• Facilitate education sessions, coaching and reflective practice across teams
• Analyse clinical data to identify opportunities for improvement and innovation
• Partner with leaders to strengthen service delivery and respond to emerging needs
• Represent the service in Uniting-wide clinical initiatives and committees

About you
• Bachelor qualification in Nursing or Allied Health
• Current registration with AHPRA (or relevant body)
• Minimum 5+ years’ experience in clinical practice, ideally within aged care or community care
• Experience in clinical governance, quality, and risk management
• Ability to coach, mentor and influence multidisciplinary teams
• Experience managing complex clinical cases in a community setting
• Strong understanding of aged care standards and contemporary practice
• Excellent communication skills and ability to navigate complex environments
• Passion for continuous improvement and social impact
• Current Driver’s Licence and willingness to travel locally
