About We Care Coordination

We Care Coordination partners with Trilogy Care, a leading self-managed Support at Home provider, to help older Australians live independently at home for longer.

We coordinate services, manage care plans and budgets, and support clients to get the most from their aged care packages with transparency, choice, and genuine value. We’re a growing, forward-thinking team that values people, innovation, and meaningful work.

What You’ll Do

  • Act as the primary point of contact for a portfolio of care recipients
  • Coordinate and schedule care and support services
  • Maintain accurate client records and detailed case notes
  • Manage and monitor budgets to prevent overspending
  • Confidently use multiple technology platforms to deliver services
  • Source information and solutions to support client goals

  • Strong interpersonal and organisational skills
  • Proficiency in Microsoft 365
  • Comfortable using multiple systems and AI tools
  • Resilient, detail-oriented, and able to prioritise tasks
  • Confident decision-maker and collaborative team player
  • Coordination or administration experience (desirable but not essential)
