Schedule : Monday - Friday; 8:30AM - 5:00PM
Reporting to the Clinical Manager, the Care Coordinator (CC) is responsible for providing case management services to an assigned caseload of patients diagnosed with substance use disorders. The CC is responsible for assessing client needs, developing, implementing and reviewing service plans, and working with Phoenix House staff and other community resources in meeting/achieving client service needs.

Phoenix Houses of California is comprised of four locations. Lake View Terrace houses our senior leadership team, an adolescent residential substance abuse program, outpatient services for youth and adults, a Wraparound team, and prevention services.
Venice Beach is an adult male-only residential substance abuse treatment program.
Orange County offers residential substance abuse treatment for women and men- housed in separate areas. Additionally, we offer adult outpatient therapy, Recovery Support Services (RSS), Family Wraparound, and Behavioral Health Intervention Services (BHIS) and mental health services in the local schools.
We also have numerous locations spread throughout California that encompasses our Corrections and Rehabilitation Division. The various sites offer mental health services to individuals in prisons, jails, or correctional settings in order to prepare them for re-entry. Locations include: Blythe, Soledad, Susanville, Calipatria, and Bakersfield.
