Care Coordinator Supervisor
The Care Coordinator Supervisor provides leadership and supervision to care coordinators, volunteers, and interns while delivering direct services to complex elderly clients, resolving escalated issues, ensuring accurate and timely documentation and reporting, supporting staff development, collaborating with internal partners, and traveling within assigned communities to fulfill program needs.

Certification/Licensure:
Preferred: LCSW or LSW Illinois Department on Aging
Minimum: N/A
