We're seeking an experienced Care Coordinator to join our growing St Vincent's Virtual & Home (SVVH) National Programs team. Our St Vincent's Virtual & Home program is growing, providing the opportunity for more people to receive high quality clinical care at home. Integrated with the St Vincent’s hospital and community care network we are a multidisciplinary team of physicians, allied health, nursing and personal care workers supporting individuals to leave hospital sooner, remain in their homes and achieve their goals. We are an experienced, supportive and dynamic team committed to the growth of quality care within the community.

Working within a centralised care coordination model, you'll support clients throughout their healthcare journey, facilitating connected care pathways based on individual needs and preferences while ensuring a seamless experience across virtual and home-based care programs.
You'll utilise your clinical knowledge, care coordination expertise and digital health platforms to manage referrals, onboard patients, coordinate services, monitor care plans and respond to clinical escalations, helping deliver safe, effective and person-centred care.
Key Responsibilities
Coordinate patient journeys including referrals, eligibility screening, onboarding and service navigation
Provide direct client care coordination and support the management of clinical escalations as required.
Manage program enquiries and referrals, assessing client eligibility and suitability.
Monitor patient care plans and support service delivery through digital health platforms.
Act as a subject matter expert and superuser for virtual care technologies and digital care systems.
Collaborate with healthcare professionals, service providers and stakeholders to deliver integrated care.
Coordinate subcontracted services where required to ensure continuity of care and positive client outcomes.
Maintain accurate clinical documentation, records, reporting and activity data in accordance with regulatory and organisational requirements.
Participate in quality improvement initiatives, service development activities and program evaluation.
Work with a high degree of autonomy while supporting national program objectives and operational excellence.
You're a skilled healthcare professional who thrives in a fast-paced, collaborative environment and is passionate about coordinating care that makes a genuine difference to people's lives.You bring strong clinical judgement, exceptional organisational skills and a commitment to delivering person-centred care through innovative healthcare models.
You will bring:
Relevant tertiary qualifications in Nursing or Allied Health.
Current AHPRA registration or membership of an appropriate professional association.
Minimum three (3) years' experience in an acute, community or relevant healthcare setting.
Strong assessment, clinical management and care coordination skills.
Experience with discharge planning and liaison with healthcare providers and community services.
Ability and willingness to work within a virtually enabled model of care.
Excellent communication and relationship management skills across patients, families, carers and healthcare stakeholders.
Strong organisational skills with the ability to prioritise, adapt and manage competing demands.
Digital literacy and confidence using healthcare technologies and data systems.
A positive, flexible and collaborative approach to work.
Knowledge of the Australian healthcare system and private health insurance environment will be highly regarded.

The Royal Children's Hospital (RCH) has been providing outstanding care for Victoria's children and their families for over 147 years.
We are the major specialist paediatric hospital in Victoria and our care extends to children from Tasmania, southern New South Wales and other states around Australia and overseas.
With a passionate, highly skilled and committed staff campus wide of over 5,000, we provide a full range of clinical services, tertiary care and health promotion and prevention programs for children and young people.
We are the designated state-wide major trauma centre for paediatrics in Victoria and a Nationally Funded Centre for cardiac and liver transplantation.
