The Care Coordinator is an experienced clinical professional who provides guidance and education to patients and families. The Care Coordinator is responsible for health management and coordination of care for a designated population of patients and exhibits a deep understanding of each patients medical needs, insurance coverage, health literacy, and other socioeconomic factors that may impact their opportunity for wellness. The Care Coordinator aids and assists patients in overcoming any barriers presented by these elements.
Position Requirements:
Associates Degree in Nursing and 2 years or more of experience in Nursing. Certification(s) required: Current American Heart Association or American Red Cross Basic Life Support (BLS). Licensure(s) required: Current Registered Nursing License (Georgia) or qualifying multi-state compact license.
Preferred Requirements: Bachelor's Degree in Nursing. 1 year or more of experience in Case Management. National Certification in Case Management.
