Care Coordinator - Refugee Resettlement Program

Lutheran Social Services of Wisconsin & Upper Michigan (LSS)

Are you passionate about helping individuals and families build new lives and achieve economic independence? Lutheran Social Services of Wisconsin & Upper Michigan (LSS) is seeking a Care Coordinator to join our Refugee Resettlement Program In this rewarding role, you will help refugees successfully transition into the workforce by developing employment opportunities, building employer partnerships, leading employment readiness workshops, and providing individualized support to clients as they navigate life in the United States.

This position is ideal for someone who enjoys teaching, building community connections, advocating for others, and making a meaningful difference in the lives of refugees and immigrants.

What You'll Do

As a Care Coordinator, you will:

  • Develop and implement individualized service plans that help refugee clients achieve employment goals.
  • Assess client needs and identify strengths, barriers, and opportunities for successful employment.
  • Build and maintain relationships with local employers to identify job opportunities.
  • Conduct weekly Employment Workshops covering topics such as workplace expectations, timeliness, communication, hygiene, paychecks, workplace culture, and job readiness skills.
  • Coordinate services and connect clients with community resources and supports.
  • Collaborate with refugee families, community partners, interpreters, and service providers.
  • Facilitate client meetings and advocate for client success.
  • Assist clients with employment onboarding paperwork and requirements.
  • Coordinate orientation dates and times with employers and arrange interpretation services for clients when needed.
  • Develop and maintain relationships with local employers to identify employment opportunities for refugees.
  • Develop and update resumes, share job leads and employment resources, and assist clients with completing employment applications when needed.
  • Maintain accurate and timely documentation in accordance with LSS, funder, and program requirements.
  • Prepare monthly and other required Excel reports for funders.
  • Complete semi-annual and annual narratives for funders
  • Work a flexible schedule that may include occasional evening and weekend hours based on client needs.
  • Report all data on behalf of refugees into designated funders' databases

What We're Looking For

Required Qualifications

  • High School Diploma or equivalent.
  • Valid driver's license, reliable transportation, and ability to meet LSS driving requirements.
  • Strong communication, organization, and relationship-building skills.
  • Ability to work effectively with individuals from diverse cultural backgrounds.
  • Proficiency with computers, Microsoft Office applications, email, and electronic documentation systems.

Preferred Qualifications

  • Bachelor's degree in Social Work, Sociology, Psychology, Human Services, or a related field or three (3) years of experience working with refugee resettlement, immigration services, workforce development, case management, or related human services programs.
  • Experience facilitating groups, leading workshops, training, or presentations.
  • Knowledge of community resources and employment services.
  • Bilingual abilities strongly preferred, especially:
    • Dari
    • Pashto
    • Rohingya
    • Swahili
    • Somali
    • Spanish

Work Environment & Travel

  • Community-based position working in client homes, offices, and community settings.
  • Frequent local travel required; some overnight travel may occur.
  • Position may require bending, kneeling, climbing stairs, and accessing various community locations.
  • Ability to respond appropriately in crisis situations and challenging client interactions.
