Are you organized, detail-oriented, and passionate about supporting individuals on their healthcare journey? Join our Primary Care Health Home team at Centerstone as a Care Coordinator, where you'll play a key role in helping ensure clients receive coordinated, high-quality care. In this position, you'll support program operations, monitor client activity and reporting requirements, and serve as an essential resource for both the Healthcare Homes team and clinical staff across the organization.

What You'll Do

  • Provide administrative and clerical support to the Healthcare Homes (HCH) Director and team.
  • Coordinate and distribute reports to the HCH team, Community Support Specialists (CSS), and community providers as needed.
  • Track client activity and monitor required screenings, assessments, and reporting measures to ensure compliance and timely submission.
  • Prepare and compile monthly reports for review by HCH leadership prior to submission to the Missouri Department of Mental Health (DMH).
  • Maintain monthly logs and documentation related to Healthcare Homes program activities.
  • Serve as a resource to team members on Care Manager, Cyber Access, and related systems.
  • Provide technical assistance and training support to HCH staff and Community Support Specialists.
  • May serve as the Cyber Access Practice Administrator, managing user accounts, monitoring agency utilization reports, and promoting the use of Direct Inform.
  • Attend required state meetings, trainings, and program updates.
  • Support quality assurance initiatives and comply with utilization review procedures and clinical records requirements.
  • Assist with special projects and other duties as assigned.

Required Qualifications

  • High School Diploma or GED.
  • Strong organizational and time-management skills.
  • Excellent verbal and written communication skills.
  • Proficiency with Microsoft Office applications.
  • Ability to manage multiple priorities while maintaining attention to detail.

Preferred Qualifications

  • Previous experience in healthcare, behavioral health, social services, care coordination, or medical office settings.
  • Experience working with data tracking, reporting, or compliance documentation.
  • Familiarity with electronic health records (EHRs) or healthcare management systems.
