Position Summary

The Care Coordinator plays a critical role in ensuring clients receive consistent, high-quality care.
This position is responsible for caregiver scheduling, client communication, and coordinating services between caregivers, clients, and management.

This role requires strong organization, communication, and problem-solving skills

Key Responsibilities

  • Coordinate and manage caregiver schedules and client assignments.

  • Ensure all client shifts are covered and staffed appropriately

  • Communicate with caregivers regarding schedule updates, call-offs, and replacements

  • Serve as a point of contact for caregivers and clients

  • Monitor caregiver attendance and shift completion through scheduling systems

  • Assist with Intake of new clients and field visits according to schedule.

  • Maintain accurate documentation of client services in HHA

  • Respond to urgent staffing issues and find coverage when necessary

  • Work closely with management to ensure high-quality service delivery

  • Support general administrative tasks as needed

Qualifications

  • Experience in home care, medical scheduling, or care coordination preferred

  • Strong organizational and multitasking skills

  • Excellent communication and interpersonal skills

  • Ability to handle sensitive and confidential information

  • Experience using scheduling software or healthcare systems (experience with HHAeXchange is a plus)

  • Proficiency in Google Workspace and email communication.

  • Ability to remain calm and solutions-focused in a fast-paced environment.

Compensation: $15.00 - $18.00 per hour
