As a key member of our metro team, you will play a vital role in ensuring clients receive timely, coordinated, and person-centered care that supports their overall well-being.
In this position, you will assess client needs through intake and preliminary screenings, connect individuals and families to appropriate services and community resources, and collaborate with internal and external partners to develop and implement effective treatment plans. You will help address Social Determinants of Health (SDOH), support referral management and scheduling, and maintain accurate, timely documentation within electronic client record systems. Additionally, you will participate in multidisciplinary team meetings, monitor Encounter Alert Service (EAS) notifications and follow up on hospital or emergency department discharges, and ensure ongoing communication across provider caseloads to promote continuity of care. This role also includes participating in rotation weeks for new client inquiries and referrals, assisting with annual paperwork and releases of information, and completing other related duties as assigned.
