About the Role
As a Care Coordinator, you'll provide comprehensive, person-centred mental health care to young people accessing Frontyard Youth Services. Working as part of the Check-In Community Mental Health Team at Frontyard Youth Services, you'll deliver high-quality, recovery-oriented care within a collaborative multidisciplinary environment, supporting young people to achieve better health, wellbeing and independence. Working within the Check-In Team, you'll complete holistic biopsychosocial assessments, develop recovery-focused care plans and coordinate services that address each young person's health, mental health and psychosocial needs. You'll manage an active clinical caseload while building strong therapeutic relationships that empower young people to develop the skills and confidence to manage their wellbeing and recovery. You'll work closely with a broad multidisciplinary team and external service providers to ensure coordinated, integrated care for some of Melbourne's most vulnerable young people. Clinical supervision will be provided by the Senior Care Coordinator and Senior Occupational Therapist (Mental Health Care Coordinator), with day-to-day operational support provided onsite at Frontyard Youth Services.
Classifications for this position will range from:
Registered Nurse Grade 4A YW17 ($116,762 per annum pro rata)
Allied Health Grade 2 ($90,236 - $103,724 per annum pro rata).
About You
You'll bring a genuine commitment to working alongside young people experiencing homelessness and mental ill-health, together with the clinical skills to deliver compassionate, recovery-oriented care.
You'll also have:
A Nursing or Allied Health qualification
Current registration (or eligibility for registration) with AHPRA where applicable
Minimum five years' experience in an acute, subacute, community and/or mental health setting
Demonstrated experience undertaking comprehensive mental health assessments and care coordination
Excellent communication, collaboration and relationship-building skills
Current National Police Check (or willingness to obtain)
Current NDIS Worker Screening Check and Working with Children Check (or willingness to obtain)
About Frontyard Youth Services
This position sits within the Healthcare for the Homeless (HfH) Program at St Vincent's Hospital Melbourne. The HfH Program provides integrated healthcare for people experiencing, or at risk of, homelessness by improving access to coordinated healthcare and addressing gaps in service delivery. Based at Frontyard Youth Services in Melbourne's CBD, the Check-In Team delivers specialist community mental health support to young people aged 12–25 experiencing homelessness or significant vulnerability. Operated by Melbourne City Mission (MCM), Frontyard is Victoria's dedicated youth homelessness access point, bringing together crisis accommodation, health and mental health services, outreach, housing support and pathways to long-term independence under one roof. Established in 2017 and expanded following the Royal Commission into Victoria's Mental Health System, the Check-In Team plays a critical role in ensuring vulnerable young people receive timely, coordinated and responsive mental health care.
