Overview

*All positions are located in Fresno/Clovis CA*

As a Limited-Time Care Coordinator with the Central Valley’s largest private employer, and at the only inpatient mental health facility in the Valley -- the average day is anything but average and you will actively feel and see how your role directly impacts our mission. As the recognized source of help for adults in need of acute psychiatric care of all spectrums, Community Behavioral Health Center is vital to improving our community’s overall well-being. Your skills combined with the training we provide, will allow you to become a leader in creating a plan of care while gaining multidisciplinary skills throughout your career.

Our Community Behavioral Health Center is licensed to 73 beds, carries an average daily census of 69 patients, and has an average length of stay of 5-6 days. Here you will be part of a close-knit all RN unit with Mental Health Workers (who have a CNA background) to assist in the delivery of daily care.

Responsibilities

Responsible for various aspects of care management to ensure that care being rendered is indicated, resources utilized are appropriate, and services provided are in the appropriate setting. Works collaboratively with the Case Management team, other departments and physicians to facilitate efficient and appropriate management of all cases.

Qualifications

Education• Bachelor's Degree in Social Work, Psychology, or related field required Experience• 2 years acute Case Management experience within the past 5 years required• Case management experience to be in a field related to Behavioral Health preferred Licenses and Certifications• BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
