Easterseals MORC is hiring for a Care Coordinator Assistant to help make a difference and become part of something bigger than yourself!

Qualifications:

  • High School diploma or equivalent.
  • One year experience working with individuals with intellectual and developmental disabilities.
  • Reliable means of transportation with proof of auto insurance and valid Michigan Driver's License

Duties and Responsibilities:

  • Monitor individual treatment and support plans, notify the appropriate I/DDCare Coordinator when needed.
  • Gather and compile personal and demographic information.
  • Understand rules and regulations governing care and support for individuals with developmental disabilities.
  • Attend planning meetings using person-centered principles and coordinate supports and community services.
  • Coordinate transitions into personal residences and broker services/supports.
  • Linking to, coordinating with, follow-up of, advocacy with, and/or monitoring of
  • supports and other community services/supports
  • Coordinate with Medicaid and other health care providers.
