The NewPath Child & Family Solutions Care Coordinator is responsible for coordinating the implementation of the Plan of Care, including a Transition and Crisis Plan, through service delivery with providers and community resources.
This position is located at our NewPath Central Campus, 4721 Reading Rd. Cincinnati, OH 45237.
