Job Location: 150 GLENWOOD LN. - BIRMINGHAM, AL 35242

Position Type: Full TimeGlenwood is a private nonprofit organization with a continuum of care for children and adults with autism spectrum disorders and children with behavioral health disorders. We are seeking a Care Coordinator to support the children and adults in our Dually Diagnosed Program. The dually diagnosed program is comprehensive and tailored for individuals confronting the unique challenges of a dual diagnosis of intellectual and developmental disabilities (IDD) alongside mental health conditions (MI).

Essential Functions

  • Comply with the Code of Ethics and demonstrate respect and regard for the dignity of all clients, families, visitors
  • Provide therapy/case management services to assigned caseloads to include: individual, group, and family therapy, crisis intervention, attending face-to-face assessments, ISP meetings, support team meetings, Court Hearings, etc. as indicated or assigned
  • Coordinate home visits and individuals’ needs with legal guardians
  • Manage the clinical record according to standards and agency protocols
  • Coordinate treatment planning for assigned cases
  • Accept and process referrals; arrange preadmission and conduct intake and discharge planning
  • Participate in psychiatric-led treatment team meetings
  • Ensure administrative and client-related paperwork is completed timely such as clinical records, administrative paperwork, UMUR clinical reviews
  • Fulfill professional development requirements and follow guidelines for ethical and professional behavior
  • Work collaboratively with other clinicians within the division and agency covering cases when able, and providing co-therapy when appropriate
  • Provide training on Clinical matters to staff and interns as assigned or accepted
  • Participate in monthly mandatory house/program and divisional meetings
  • Share on-call responsibilities with other qualified mental health professionals.
  • Provide direct care, staff support, and supervision as needed

QualificationsSkills and Abilities

  • Clinical skills in the assessment individual, group and family therapy
  • Strong interpersonal, communication skills
  • Ability to work effectively with a diverse population
  • Relates well with referral agents
  • Excellent administrative skills
  • Excellent written and oral communication skills, use of computer systems
  • Organizational skills, attention to detail (Interpersonal, time management and problem-solving skills, and the ability to work well within teams are essential)

Education and Licensure

  • Master’s degree in social work or other related fields
  • ALC, LPC or LGSW, LCSW

Minimum Experience

  • 3 years’ experience preferred in clinical, mental health setting, licensed or license eligible
