We seek a Care Coordinator to join our Behavioral Health Urgent Care (BHUCC) team in Philadelphia, Pa. This posting is Full-time, First Shift (Monday - Friday 10:30am - 7:00pm EST).
The hourly rate is $29/hr. as it includes a $2/hr shift differential!
We are a Behavioral Health Urgent Care/Walk-In Center for adults needing non-crisis same-day care in Philadelphia. Consider it “urgent care” for individuals experiencing emotional or behavioral health concerns. Individuals will participate in resolution-focused services and aftercare planning to prepare for long-term recovery and connection to ongoing care. We serve individuals regardless of income or insurance status. Our services do not replace the need for ongoing mental health care; rather, we aim to reduce avoidable visits to crisis centers and emergency rooms by helping people get the mental health care they need when they need it.
Position Details
The Care Coordinator serves as a floor leader within the urgent care setting, overseeing patient flow to ensure individuals move efficiently through the clinic’s workflow and crisis response protocols. This role is responsible for coordinating the initial engagement and triage processes, while ensuring all required information is accurately documented in the electronic health record (EHR). Operating in a fast-paced, dynamic environment, this position promotes a person-centered experience aligned with a “living room” model of care, ensuring individuals feel supported and informed while awaiting urgent services.
