
The Care Manager Specialist has overall responsibility and accountability for coordinating all aspects of assigned heath home member’s care. The Care Manager will track and arrange appointments, educate members and coordinate other aspects of the member’s health and community services. Responsibilities include but are not limited to: assisting members with consent forms, conducting outreach and engagement activities, conducting initial and periodic needs assessments, assisting with the development and execution of care plans, providing links to services and communicating among care management team, providers, members and families caregivers.
Additional Requirements
Must have minimum of two years’ experience in Healthcare, Managed Care setting or related field and,
Education Requirements: Bachelor’s degree required; Master’s degree preferred
Valid driver’s license in good standing
Proficient in Microsoft Office Suite (Outlook, WORD, Excel, PowerPoint) and database navigation required.
