Your new role:
Due to significant growth of new RSL LifeCare clients requiring our support, we have a Care Coordinator position available in Canberra for our Home Care team.
You will be a key team member helping to deliver a variety of services to meet our client’s unique circumstances and needs ranging from entry level through to complex health needs.
With a large client base, you will oversee a portfolio of clients, develop and manage care plans, as well as create services and supports which are unique to individuals.
Administratively, you will be responsible for a range of tasks such as drafting budgets, monitoring financial transactions, actioning changes in care, undertaking general marketing, as well as working with staff to deliver exceptional services.
Essential Criteria
Demonstrated experienced in Support at Home / Aged Care funding
Current CPR Certificate
Solid understanding of client centred, community-based care service delivery and associated quality standards
Experience working with and adhering to budgets
Ability to liaise with and promote our services to external providers
Ability to create and maintain interpersonal relationships with staff
Current drivers’ license with own fully insured vehicle
Proof of Covid-19 vaccination status

It is a mandatory requirement for a team member working in the Aged Care sector to have a valid Police Check or a valid National Disability Insurance Scheme (NDIS) check, or potentially both, to undertake the inherent requirements of the role. The Royal Children's Hospital (RCH) has been providing outstanding care for Victoria's children and their families for over 147 years.
We are the major specialist paediatric hospital in Victoria and our care extends to children from Tasmania, southern New South Wales and other states around Australia and overseas.
With a passionate, highly skilled and committed staff campus wide of over 5,000, we provide a full range of clinical services, tertiary care and health promotion and prevention programs for children and young people.
We are the designated state-wide major trauma centre for paediatrics in Victoria and a Nationally Funded Centre for cardiac and liver transplantation.
When it comes to training and research we partner with the very best. Our campus partners, the Murdoch Childrens Research Institute (MCRI) and The University of Melbourne Department of Paediatrics, along with the RCH Foundation, are on site with the hospital in Parkville.
