As a Care Coordinator, you will assess and determine patient care needs and eligibility, provide access and referrals to community services, and engage with patients, caregivers and other health care practitioners.

All hours of operation, including extended hours (70 hours per bi-weekly pay period). Subject to change as per the Collective Agreement
Position location and travel:
Regular travel within the geographical region of Ontario Health atHome Central East may be required. A valid Ontario driver’s license and access to a vehicle are necessary.
