Job DetailsJob Location: Health Homes - Staten Island, NY 10301Position Type: Full TimeEducation Level: NoneSalary Range: $22.42 - $24.83 HourlyJob Shift: DayRESPONSIBILITIES:
1. Provide intensive care coordination activities for members.
2. Assuring that member has access to, engages in and retains needed services as defined in the member’s
care plan. Such services may include: Acute Medical Care, Primary Medical Care, Preventative medical
care Services, Home Health Care, Behavioral Health Services, Community Social Support Services,
Housing & State and Federal entitlements.
3. Advocate for member services by phone and in person as needed.
4. Perform home visits and follow-up to monitor services and member status.
5. Participate in case conferences and trainings.
6. Maintain relationships with service providers.
7. Maintain accurate and timely documentation including all documents included in member charts, records
and billing activity logs.
8. Attend supervision with Program Director as per agency policy.
Qualifications
Requires a High School Diploma or GED with 2 years of care management experience.
Experience working with the homeless, and HIV populations required.
Bilingual preferred, with good verbal skills in Spanish.
Must have exceptional communication skills both orally and in writing, with individuals and groups regarding complex and sensitive issues.
Clean valid driver's license required.
