Under the direction of the Director of Health and Wellness, the Care Coordination Supervisor/LPN is responsible for ensuring that quality healthcare coordination and nursing services are provided to youths placed in HSVS foster boarding homes (FBH) and foster care group residential facilities (Healing Homes) in Queens and Staten Island. The Care Coordination team uses a trauma-informed approach to ensure that all HSVS foster care children and young adults are receiving all needed medical, dental, behavioral health, and specialty health services through HSVS’s 29-I primary care clinic and Medicaid Managed Care Plan (MMCP) in-network providers, and that these services are being documented appropriately in the NextGen Electronic Health Record (EHR). The Care Coordination Supervisor/LPN is an integral part of this growing Care Coordination team and will also work closely with the department’s Managed Care Liaison team. Clinical oversight will be provided by the 29-I Registered Nurse Supervisor and 29-I Medical Director, as appropriate.

Primary Responsibilities and Essential Functions of Position:

  • The Care Coordination Supervisor/LPN will:
  • Supervise FBH Care Coordinators and the Queens and S.I. Care Coordinator/LPNs in the day-to-day coordination of all healthcare services for their assigned caseloads, with the goal of ensuring overall quality of care and timeliness in service delivery
  • Provide/ensure caseload coverage during supervisee's absences
  • Provide clinical oversight to Care Coordinators, within scope of practice
  • Under clinical oversight by the 29-I Registered Nurse Supervisor, as needed, review medical exam records for further treatment recommendations, including referrals and medications.
  • The Care Coordination Supervisor will manage twenty cases from the (FBH) Foster Boarding Homes unit.

Coordinate all aspects of healthcare services for a personal caseload of higher needs foster care youths including, but not limited to:

  • Act as liaison/point person for healthcare providers and pharmacies
  • Schedule and ensure attendance at medical/dental/behavioral health appointments, per ACSs schedule of required regular evaluations; help coordinate transportation to appointments as needed
  • Monitor and address youths' healthcare needs in between routine primary care visits, including facilitating access to urgent care
  • Ensure timely follow-up on referrals, emergency care and hospitalizations
  • Conduct youth assessments, including HIV, STD, and substance abuse risk assessments
  • Ensure family medical history, proper consents (routine, mental health treatment, psychotropic medication, etc.), HIPAA authorizations, and all other required medical record documentation are entered in NextGen EHR
  • Complete additional required actions in NextGen EHR, including processing 29-I clinic referrals, submitting referrals to HSVSs Article 31 behavioral health clinic, CHH Warm Hand-offs, data share reconciliations, etc., as applicable.
  • Coordinate closely with HSVSs 29-I clinic, Healing Homes, Case Planning, and Children's Health Homes teams, as applicable, in the provision of above services and to establish treatment goals.
  • The Care Coordination Supervisor will complete the Medical Necessity form and ensure that the RN is tasked and signs the document for OCFS compliance.
  • The Care Coordination Supervisor will perform quarterly visits to the Healing Homes. There are two Healing Homes in Queens and two in Staten Island; every three months, a different Healing Home will be visited.
  • Perform periodic physical body checks for abuse, as back-up to 29-I clinical staff.
  • Develop a trusting caregiver relationship with foster care youths and families

​​​​​Ensure staff completion of the following is documented:

  • Processing of new foster care intakes upon placement per ACS/29-I requirements, including required 30-day initial medical, dental, behavioral health assessments, and required timely documentation of Medical Necessity and the Comprehensive Treatment Plan in NextGen EHR.
  • Processing of discharges, including timely final discharge medical exam and providing required medical records from NextGen EHR.
  • Ensure the completion of twice-yearly adolescent assessments for all foster care youths age 12yrs+: sexual/reproductive health, gender identity, sexual orientation, and family planning counseling.
  • Coordinate with Managed Care Liaison team to ensure that healthcare appointments are scheduled with providers in-network with child's Medicaid Managed Care Plan (MMCP).
  • Attend, and ensure that Care Coordinators attend, all required meetings and trainings.
  • Work with the Director of Health and Wellness to facilitate departments ongoing medical chart self-audits.
  • Engage in ongoing review of departmental policies, procedures, and practices to make recommendations for quality improvement.
  • Perform other tasks as assigned within the reasonable scope of duties of the position.



Qualifications

  • NY State LPN license and current registration required
  • Associate degree or equivalent nursing experience required
  • At least one year of supervisory experience
  • One or more years case management experience in a healthcare environment and/or in the child welfare system
  • Knowledge of:
    • the child welfare system and the unique complex needs of the foster care population
    • foster care healthcare requirements
  • Experience documenting in Electronic Health Records (EHR)
    • NextGen EHR experience a plus
  • Ability to provide education about non-routine medical and surgical procedures
  • Ability to facilitate training or coaching around medical conditions and treatment
  • Willingness to travel to Staten Island and Queens to do periodic site visits
  • Excellent written, oral, organizational, and interpersonal communication skills, and be a collaborative team player
  • Proficiency in Microsoft Office, including Outlook and Excel
