We’re looking for an Integrated Healthcare Case Manager to join our Care Coordination team at The Perry Hall in Pueblo.
What You’d Be Doing
As the Integrated Healthcare Case Manager, you would play a central role in helping our patients access services and take the next steps to wellness. Your work would be primarily in the office with the possibility of some travel to healthcare facilities and patients’ homes. You would be called upon to collaborate with other professionals within and outside our agency to assure that clients receive the best care available.
Specifically, the Case Manager
- Coordinate and support patient care activities to promote continuity, access, and quality outcomes across services
- Assess and respond to patient needs by addressing clinical, behavioral, and social factors that may impact health and well-being
- Facilitate connections to appropriate internal services and community resources to support comprehensive care
- Support patient engagement and access to services, helping to ensure timely utilization of care and adherence to treatment plans
- Collaborate with interdisciplinary teams and external partners to ensure coordinated, integrated service delivery
- Serve as a point of communication between patients, providers, and community organizations to support continuity and alignment of care
- Contribute to the achievement of organizational goals, performance metrics, and quality initiatives
- Maintain accurate and compliant documentation in accordance with organizational, regulatory, and professional standards
- Participate in cross-functional activities and team initiatives to support operational needs and program effectiveness
- Assist in the development and improvement of workflows, processes, and best practices to enhance efficiency and service delivery
- Identify opportunities for improvement and support problem-solving, innovation, and quality enhancement efforts
- Foster effective communication and collaboration across departments to support a team-based approach to care and operations
- Perform additional duties as assigned to support program growth, patient care, and organizational priorities
Physical requirements include the following
- Requires standing, sitting, driving, and walking for prolonged periods of time
- Occasionally lifts and carries items up to 20 pounds
- Requires working under stress in emergency situations
- Requires normal range of hearing and eyesight to record data electronically and communicate with clients
- Requires occasional driving in adverse weather conditions
- Requires frequent use computer operation including keyboard and mouse

QualificationsWhat We’re Looking For—The Must-Haves
- Baccalaureate degree in social work, clinical psychology, healthcare management, human services, or a closely related field
- Computer proficiency
- Colorado driver’s license, a good driving record, and access to transportation during work hours
What We’d Like to See in You—The Nice-to-Haves
- Experience successfully working in multi-disciplinary teams
- Experience working with adults with severe and persistent mental illness
- Experience providing case management services to adults
- Community systems navigation background
- Spanish language skills
- CAC training
- Experience working with those experiencing co-occurring disorders
- Knowledge of and experience with HUD housing rules and regulations
