Greater Nashua Mental Health is seeking a Care Continuity Coordinator to join our Emergency Services team. This position serves as a critical liaison between Greater Nashua Mental Health, New Hampshire Hospital, designated receiving facilities, local hospitals, and treatment teams to ensure timely admissions, discharges, care transitions, and continuity of treatment for clients across the behavioral health system.

Key Responsibilities

  • Act as the primary liaison between Greater Nashua Mental Health and New Hampshire Hospital.
  • Coordinate referrals and discharges from New Hampshire Hospital to GNMH services through completion of releases, record review, care coordination, and treatment team engagement.
  • Engage with the New Hampshire Hospital Care Traffic Control team regarding admission and discharge concerns requiring administrative support.
  • Coordinate emergency inpatient admissions and discharges with local designated receiving facilities.
  • Maintain and monitor a list of clients subject to court-ordered outpatient treatment and support treatment teams with renewals and compliance requirements.
  • Support transfers of conditional discharge orders and outpatient orders both to and from the Region VI catchment area.
  • Review local hospital census information daily and notify treatment teams of client admissions and discharges.
  • Review Hillsborough House of Corrections inmate census information and notify treatment teams of incarcerations and releases.
  • Track and coordinate transfers of clients to other catchment areas.
  • Perform additional duties and special projects as assigned.

Qualifications

  • Bachelor's Degree required; Master's Degree preferred.
  • Field of study in Counseling, Social Work, or a related mental health discipline.
  • 1 to 2 years of relevant experience preferred.
  • Strong understanding of the New Hampshire mental health system and hospital discharge processes.
  • Excellent collaboration, communication, and case presentation skills.
  • Ability to develop and maintain tracking systems related to legal commitments and treatment transitions.
  • Knowledge of Adobe and Microsoft Office products including Outlook, Word, and Excel.
  • Valid driver license, reliable transportation, and insurance required.

Salary Range: $23.84/hr - $31.52/hr
