Care Coach
The Care Coach provides support and guidance to assigned patients in an effort to help them become engaged in managing their own health and care needs. Follows defined care guidelines based on the patient's health conditions and collaborates with other caregivers and community agencies to provide optimal support.

Hours: Full-time, 40 hours/week
Required: HS Diploma; CNA or completion of 1st semester RN Program
Pay: Based on experience, starting at $16.94 per hour
Responsibilities
Administers a health screening survey to potential care coordination program participants to determine support needs., Assists in the implementation of quality improvement initiatives., Communicates with program participants and their caregivers to educate and encourage self-care using established care guidelines., Demonstrates familiarity with community resources and provides appropriate referrals., Demonstrates sound judgement in relaying timely and appropriate information to Care Coordinators., Establishes a positive, professional relationship with patients, caregivers and community agencies., Follows established care guidelines applicable to assigned patients., Identifies opportunities and recommends methods to improve service, work processes and financial performance, e.g. program cost savings and other quality improvement initiatives., Initiates outbound calls to patients, providers and/or caregivers., Maintains complete and current documentation as required., Makes visits to patient homes for follow-up. Checks on patient compliance with prescribed medications, physician orders, attendance at appointments, etc., Prioritizes and addresses requests and assignments in a professional manner to develop cooperative relationships and to ensure customer confidentiality at all times., Provides support and guidance to assigned patients in an effort to help them become engaged in managing their own health and care needs.
