Job Description
Assist with resident care under the direction of Licensed Personnel and within the scope of defined practice. Demonstrates a commitment to professional accountability and growth to maintain and expand knowledge and skills. Serves as a liaison between Resident Services Manager, licensed staff, outside health services and families ensuring communication and accurate records are available. Adheres to practice in accordance with the Ephraim McDowell Health nursing professional practice model. Performance that accomplishes patient and family centered care as central to practice and a foundation of caring, compassion, competence, communication, collaboration and teamwork. Responsible to participate in shared nursing leadership to accomplish positive outcomes, education, respect, dignity, and safety. Essential Job Duties
- Provides direct resident assistance with ADL’s including self-administration of medications, bathing, dressing, eating, grooming, transferring and toileting according to a posted schedule and under the supervision of the Resident Service Manager and/or LPN.
- Provides direct resident assistance with any IADL's such as laundry, bed-making and other clerical functions including phone calls to families, physicians, pharmacies, home health agencies or hospice, faxing, etc.
- Evaluation and follow-through on all resident emergency situations to ensure resident is stabilized until EMS arrives or the situation is resolved.
- Completes incident reports on all occurrences and contacts family/responsible party to report occurrence in a timely manner. Communicates incident to Resident Services Manager.
- Implementation and involvement in all emergency procedures such as fire drills and actual responses, weather related emergencies, etc. to ensure resident safety.
- Participates in meal service for all resident meals and snacks; assisting residents, serving meals, table clean-up and setting dining room for upcoming meal.
- Ensures all residents are served in the dining room or in their apartment.
- Maintains a neat and orderly resident apartment including daily trash removal and soiled linen pick-up.
- Performs daily cleaning duties, as assigned and scheduled.
- Reminds/escorts residents to and from activities; leads and assists with activities on a regular basis.
- Assists with glucose testing per monitor and records results as scheduled. Cleans monitor per protocol.
- Empties urinary catheters as needed.
- Assists with nebulizer treatments as scheduled.
- Takes vital signs including measures such as blood pressures, weights, pulse and pulse/ox and records in a timely manner for evaluation by Licensed Personnel and/or physician.
- Assists with scheduled self-administration of medication via various routes including oral medications, topical ointments, ear, nasal, and eye drops and hand-over-hand technique in accordance with Kentucky Assisted Living statutes and regulations.
- Observes compliance with medications, oxygen and therapy exercise regimens as well as communications with Licensed Personnel and assists the residents as necessary.
- Documents assistance with self-administration of medications in a timely organized manner. Maintains medication record book.
- Nightly resident rounds to ensure resident safety and oxygen/CIPAP compliance if applicable.
- Provides care for residents receiving hospice services in collaboration with hospice nurses and other care staff.
- Maintains accurate and complete resident records to ensure expeditious retrieval of information.
- Participates in shift report and documents in communication log as needed.
- Distributes newspapers to residents’ apartments.
- Provides resident specific laundry services and bed making services according to the daily schedule.
- Completion of monthly chart audits.
- Communicates directly with residents, families and outside service providers to ensure continuity of care between disciplines.
- Demonstrates communication skills appropriate for Assist Living Community.
- Prioritization to meet resident needs.
- Knowledgeable of and practices according to McDowell Place policies and procedures (compliance with assist with self-administration of medications).
- Assesses age specific resident safety issues (National Patient Safety Goals, Safe Patient Handling, safe environment, Restraints, etc.).
- Demonstrates a commitment to incorporating the core concepts of patient family centered care into practice (dignity, respect, information sharing, and participation).
- Demonstrates a commitment to the development and implementation of shared governance across the Organization.
- Considers the age of the patient served, applies knowledge of the geriatric population in planning and implementing assistance.
- Performs other related duties as assigned.
Working Conditions, Hazards and Physical Effort
Works in a well-lighted, ventilated and air conditioned area. Often exposed to unpleasant odors, sights, and sounds. Long periods in a stationary position and moving around the unit are required. Risks exposure to disease, infection and physical strain, and assault. Risks exposure to blood and body fluids. Must be able to move or position 50 lbs. independently.

Qualifications
One (1) year experience preferred.

Education Required
High School Diploma or G.E.D. required. Current certification through an approved State Registered Nursing Assistant program is preferred. If certified, must maintain 12 CEU's on an annual basis.
