Cardiology Nurse Practitioner (NP) or Physician Assistant (PA)
Siren Medical | Community-Based Cardiology
Siren Medical is seeking a clinically strong and independent Cardiology Nurse Practitioner/Physician Assistant to manage complex cardiovascular patients in a collaborative, high-accountability environment.
This role is ideal for an NP or PA who is confident managing chronic cardiac conditions, values communication with referral partners, and thrives working independently while aligned with a strong clinical team.

What You’ll Do
• Independently manage hypertension, CAD, CVA, dyslipidemia (HLD), PVD, PAD
• Oversee and adjust anticoagulation therapy, including Coumadin
• Perform comprehensive cardiovascular assessments and medication management
• Coordinate with internal providers and external referral partners
• Maintain clear communication with community stakeholders
• Deliver evidence-based, outcomes-focused care

What We’re Looking For
• Active, unrestricted license in state of practice
• 2+ years of cardiology or advanced chronic disease management experience
• Strong diagnostic and clinical decision-making skills
• Confident independent operator
• Excellent communication and referral relationship management skills
