Capacity Coordination Specialist

3x12 hour shifts D/N rotation including weekends

The Capacity Coordination Center manages overall capacity at MHMH to include bed placement at MHMH, optimization system capacity through transfer center redirect process, and monitoring capacity projections at MHMH. Works in collaboration with physicians and others in the Nursing Department to optimize patient placement flow in an effort to maximize Hospital capacity and RN to process all requests for patient transfers into DHMC, telemedicine service encounters, and provider to provider patient consults. Assumes principal responsibility and accountability for the accurate placement of patients following organizational and regulatory procedures. Independently handles telemedicine encounters, clinic/internal admissions and consult requests and helps facilitate completion of transfer requests as delegated by the CCC RN, including facilitation of alternate patient disposition. Documents actions per unit standards

Responsibilities

  • Collaborates with others to maximize day-to-day bed flow management. This includes, but is not limited to monitoring, interacting with, and ensuring accuracy of electronic patient placement systems data, providing data to CCC RN/House Supervisor/On-call Medical Director for patient placement discussions and capacity decision making
  • Implements standardized and consistent patient placement processes across all areas of responsibility. Using knowledge of the facility and placement policies, assigns locations/beds for patients.
  • Arranges external, and facilitates internal, transfers following established policies and procedures
  • Establishes timely communications between referring provider, accepting provider, CCC RN, House Supervisor, on-call medical director, unit charge RNs, and DHART as applicable
  • Maintains and updates patient transfer and consult queues, Maintains accurate and timely documentation relating to all calls and assists in maintaining databases. Obtains pertinent documentation needed to create medical record numbers
  • Implements and oversees emergency and electronic back-up methods and procedures for business continuity.
  • Assumes responsibility for operations as well as accountability for quality assessment of departmental output.
  • Provides orientation, direction and training to assigned staff members. Provides feedback to the supervisors.
  • Role models and insures implementation of all customer service standards as well as professional communication and conduct hospital-wide.
  • Demonstrates effective interpersonal skills and ability to work effectively with others as a team player. Develops and promotes long-term relationships with inpatient units, referring facilities, House Supervisor team and providers
  • Demonstrates knowledge of, practices, and communicates DHMC mission, policies, and procedures.
  • Performs other duties as required or assigned.
  • Qualifications

    • Associate’s degree with 2 years admission or office experience preferred.
    • Medical terminology required.
    • Computer capability and electronic system data entry skills required.
    • EMT, LNA or MA desirable
    • Comfortable with the use of multiple software platforms and multi-line phone systems
    • Exemplary interpersonal communication skills and demonstrated team player skills required.
    • Must be capable of performing accurately and calmly in a fast-paced environment
    • Proven organizational and multi-tasking abilities along with strong customer service skills that include excellent phone etiquette.
    • Must be capable of performing work in a multitasking environment and retain detailed information.
