The representative will work closely with the Director of Outpatient Services to identify appropriate patients for step-down levels of care. This role includes serving as a patient navigator, facilitating smooth transitions from higher levels of care to outpatient services. Responsibilities also include coordinating step-down placements with social workers and clinical teams, verifying insurance benefits and coverage, and ensuring all financial clearances are completed in a timely manner.
In addition, the representative will actively promote the program within the community by building and maintaining relationships with referral sources, healthcare providers, and community partners to support program growth and patient access to care.
salary between $58,000 - 79,000.
