Ambercare Hospice is seeking a compassionate and relationship-focused Bridge Navigator to join our team in New Mexico. Primary Purpose

To build trusted, ongoing relationships with patients and families through proactive outreach and education, helping them navigate serious illness, understand their care options, and access the right level of support at the right time. The Bridge Program Navigator promotes early goals-of-care discussions and facilitates timely transitions across the care continuum, including hospice, to improve patient outcomes, quality of life, and length of stay when hospice care becomes appropriate.

Essential Responsibilities

  • Conduct daily outbound telephone outreach to patients and families.
  • Develop and maintain ongoing relationships with assigned patients throughout their care journey.
  • Assess patient and caregiver needs, concerns, and understanding of the patient's condition.
  • Facilitate goals-of-care conversations and explore patient values, wishes, and preferences.
  • Monitor for changes in condition, functional decline, symptom burden, increased caregiver stress, or other indicators of escalating care needs.
  • Contact hospitalized patients and families to provide support and identify discharge planning needs.
  • Coordinate communication between patients, families, physicians, home health clinicians, palliative care providers, hospice teams, and other healthcare partners.
  • Identify patients who may benefit from additional supportive services and facilitate appropriate referrals.
  • Educate patients and families regarding available care options, including palliative care, hospice, personal care services, and community resources.
  • Track patient outcomes, hospitalizations, emergency department visits, and care transitions.
  • Document all patient interactions and care coordination activities accurately and timely.
  • Collaborate with interdisciplinary teams to ensure continuity of care and reduce gaps in service.
  • Support organizational goals related to early identification of patient needs, care transitions, patient satisfaction, and quality outcomes.
  • Other duties as assigned.

Qualifications:

  • Be eighteen (18) years of age or older
  • Associate’s degree in marketing, business administration, or other related fields is preferred. Acceptable combination of experience and/or training may be considered in lieu of formal education.
  • Graduate of an accredited LPN/LVN program with current license to practice in the state, preferred, with current CPR Certification.
  • At least two years’ experience in a homecare, hospice, and/or healthcare related field preferred.
  • Demonstrates good verbal and written communication, and organization skills.
  • Ability to complete necessary documentation appropriately, legibly, and timely.
  • Strong technology skills required.
  • Ability to handle stressful, delicate, and confidential situations.
  • Strong customer service skills and ability to keep good relationships with patients, families, co-workers, and other business partners.
  • Valid driver’s license with an automobile that is insured in accordance with state requirements.
