The Prevention Services Division (PSD) is actively building a workforce reflective of the diverse communities we serve. The Health Promotion and Chronic Disease Prevention (HPCDP) Branch in Prevention Services Division (PSD), reviews, plans, designs, implements and evaluates public health chronic disease interventions and related risk factors with the goal of advancing health and racial equity and improving health-related quality of life for all Coloradans. Special emphasis is placed on evidence-based practices, data-based and evidence-informed decision making, capacity-building, partnerships, and integration between program efforts for efficiency and effectiveness. The statewide chronic disease programs within the HPCDP Branch include Heart Disease and Stroke Prevention, Comprehensive Cancer Control, Colorectal Cancer Control, Diabetes Prevention and Management, Alzheimer’s and Related Dementias (ADRD), Healthy Eating and Active Living, the Cancer, Cardiovascular and Pulmonary Disease Grants Program and the Tobacco Education, Prevention and Cessation Grants Program.
We are aware that many chronic diseases share the same systemic causes: tobacco use, lack of access to nutritious foods, built environments to support physical activity, social connectedness, and poverty and economic instability. These systemic injustices and additional risk and protective factors are shared across many of the outcomes we address. Our work continues until all Coloradans have the opportunity to live and have healthy lives, in a way that celebrates intersectional identities, including race, ethnicity, gender identity, sexual orientation, education level, age, language, religion, ability, and geographic location.
This position is housed in the Diabetes, Cardiovascular Health and Alzheimer’s Disease and Related Dementias Unit (CarDiADRD Unit), which focuses on engaging with health systems, community-based organizations, and other partners to improve chronic disease outcomes for Coloradans, particularly those impacted by historical injustice and unequal distribution of resources. The unit also collaborates with internal programs, including those focused on cancer, healthy eating and active living, tobacco, informatics, among others, to support a comprehensive system of partnerships that address specific chronic disease needs in innovative and sustainable ways.
The Brain Health Generalist is responsible for coordinating and implementing activities related to Alzheimer’s Disease and Related Dementias (ADRD), including support for the Dementia Respite Education and Access Model (DREAM), implementation of ADRD-related Rural Health Transformation Program (RHTP) activities, and coordination across Colorado ADRD State Plan initiatives. This position manages contracts and project implementation activities related to clinician education, respite coordination, stakeholder engagement, technical assistance, and statewide partnership development. The position supports implementation of evidence-based and community-informed strategies that improve dementia awareness, clinician education, respite access, and coordination of ADRD-related initiatives across Colorado.

Six (6) years of relevant experience in public health, healthcare, human services, aging services, community-based programs, or a closely related field, that included program or project management, contract or grant management, stakeholder engagement, and/or implementation of related programs or activities.
A combination of related education with a degree in public health, health sciences, healthcare administration, social work, human services and/or relevant experience in public health, healthcare, human services, aging services, community-based programs, or a closely related field, that included program or project management, contract or grant management, stakeholder engagement, and/or implementation of related programs or activities equal to six (6) years.
