The transplant coordinator is an RN who is responsible for the coordination of care for transplant patients, living donors, and their families. He/She is a key member of the multidisciplinary team who facilitates the movement of the patient and family through the transplantation or living donation process by providing individualized education, support, and coordination of services across the continuum of care. This role is responsible for clinical aspects of the transplant program including but not limited to education, patient care coordination and performance improvement for program development.
Patient Care Coordination: Facilitates the medical, surgical, financial, psychosocial, nutrition and pharmaceutical evaluation and ongoing care needs for transplant candidates/recipients and living donor patients throughout the phases of transplantation. Provides education to patients and families regarding the process and maintains open communication with all parties involved.
Communication: Functions as part of the multidisciplinary team; participates in patient care planning and selection conference. Facilitates communication between team members as needed to ensure all team members are aware of the plan of care. Demonstrates the ability to critically think through patient issues and escalates concerns when appropriate.
Patient Advocate: Advocates on behalf of the patient to ensure transplantation or donation is an appropriate option. Delivers patient and family-centered care. Facilitates patient care as a case manager for transplant patients and living donors during all phases of transplantation (pre-operatively, inpatient, discharge, post-discharge and for the life of the transplant) as determined by the multidisciplinary transplant team. Provides accurate and timely documentation of care.
Documentation: Adheres to documentation policies and procedures. Knowledgeable of HIPAA regulations and follows all security policies. Updates data in OTTR and the EMR in a timely manner. Works with outsides hospitals and physician offices to obtain up to date medical records and provide updates on patient health status. Maintains compliance with regulatory standards for timely communication to patients and referring providers.
Performance Improvement: Participates in quality monitoring and process improvement activities and programs. Knowledgeable of regulations related to transplantation and living donation and follows program policy, procedures, and guidelines to ensure compliance.

Associates Nursing or Diploma (Nurse) Nursing or Bachelors Nursing-Preferred
All certifications are required upon hire unless otherwise stated.
BLS-I - Basic Life Support - Instructor or BLS - Basic Life Support or ARC-BLS - Amer Red Cross Basic Life Support
RN-COMPACT - RN - Multi-state Compact or RN - Reg Nurse (Single State)
Minimum 3 years nursing Required
