Job Description

Job Location: 520 N Columbus Blvd - Philadelphia, PA 19123
Salary Range: $43,000.00 - $46,000.00 Salary/year

Summary of Position: Targeted case management program works with individuals who are living with a serious and persistent mental illness and substance use disorder offering support with resources, linkage and connecting to community resources. Case Managers assist with accessing appropriate behavioral health services, and locating and obtaining services specified in the treatment or services plan with the goal of obtaining and maintaining independency.
Essential Position Responsibilities: (Other duties as assigned)
Coordination of meetings and necessary evaluations for participants
•Coordinate access to medical and/or other treatment services as needed by participants
•Responsible for participant/family outreach in matters surrounding the coordination of services
•Responsible to assist participant and families in accessing community support and resources
•Responsible to advocate on behalf of participant’s needs
•Responsible to assure ongoing communication and integration of services among all service providers and support systems involved with the child, adolescent, adult, and family
•Responsible to monitor the implementation of clinical strategies as developed with team

QualificationsMinimum

Qualifications
• Bachelor’s Degree in a mental health discipline or related human services field AND 2 years’ experience proving Targeted Case Management services in Philadelphia OR Master’s degree in human services, psychology or Social Work.
• Experience working adults with serious mental health AND substance use disorders
• Valid Driver’s License (issued by state in which employee lives), safe driving record and proof of insurance coverage
• Experience navigating multi-system services and a working knowledge of available community resources
• Must demonstrate a genuine respect for all adults and families
• Demonstrate an understanding of cultural competence and the principles of recovery
• Adult and Pediatric CPR Certification Required

What is a Community Umbrella Agency (CUA)? Community Umbrella Agencies (CUA’s) are community-based agencies that are responsible for providing direct case management services to families in their designated region. The CUA’s ensure that local solutions and resources are more accessible to children and families. Through community engagement initiatives, CUA staff develop connections to formal and informal neighborhood networks that can strengthen and stabilize families. They also recruit and retain foster and adoptive parents in the neighborhoods where children live.
