What You’ll Do
Clinical Care & Community Impact
- Provide comprehensive behavioral health services including psychiatric evaluations, and individual, group, and family therapy
- Deliver flexible, community-based care tailored to meet clients where they are
- Collaborate with partner sites to ensure culturally responsive care and help bridge gaps between families, agencies, and community resources
- Serve as a cultural and clinical bridge for monolingual Spanish-speaking clients
Program Development & Leadership
- Help shape and enhance culturally responsive behavioral health programming
- Collaborate with clinical leadership to define workflows, performance measures, and service delivery strategies
- Supervise and mentor a Bilingual Community Health Worker, supporting outreach and behavioral health education efforts
Care Coordination & Documentation
- Develop and maintain individualized treatment plans and clinical records
- Ensure timely documentation, assessments, and re-authorizations
- Actively participate in integrated care planning and multidisciplinary collaboration

QualificationsMINIMUM QUALIFICATIONS:
- LCSW and minimum of two years related experience serving individuals with Prolonged Mental Illness.
- At least one year of experience working with culturally diverse populations.
- Must be fluent in English and Spanish.
