Guardian Angel Senior Services is seeking a compassionate, organized, and experienced Registered Nurse (RN) to join our growing Home Care team. What You'll Do

As a Home Care RN, you will play a vital role in ensuring our clients receive exceptional care by:

• Completing initial client assessments and reassessments
• Developing and updating individualized care plans
• Supervising Home Health Aides, CNAs, and caregivers
• Conducting routine supervisory and quality assurance visits
• Providing client and caregiver education
• Coordinating care with physicians, families, and interdisciplinary team members
• Ensuring compliance with state regulations and agency policies
• Accurately documenting visits and clinical notes

What We're Looking For

• Current Registered Nurse (RN) license
• Home care, hospice, or community health experience preferred
• Strong assessment and critical thinking skills
• Excellent communication and organizational abilities
• Ability to work independently while collaborating with a supportive team
• Reliable transportation and valid driver's license
• Proficiency with electronic documentation
