
Woodfords Family Services is seeking compassionate and qualified Behavioral Health Home (BHH) Coordinators to join our growing team in Central Maine! These are full-time, salaried positions offering a comprehensive and competitive benefits package.
This role is based out of our Manchester, ME office and includes required travel throughout Kennebec County to support individuals and families in their communities.
The Behavioral Health Home (BHH) Coordinator plays a vital role in supporting children with disabilities and their families. This position focuses on providing advocacy, guidance, and connections to community-based services that promote family empowerment and long-term self-sufficiency. Support is delivered through comprehensive assessments, care plan development and monitoring, and active participation in family and team meetings.
Key Responsibilities
Conduct regular visits with children and families in various settings to evaluate functioning, monitor progress, assess quality of life and safety, and address concerns.
Maintain strong knowledge of local resources and assist families with appropriate referrals.
Complete strengths and needs assessments and facilitate wraparound family meetings as needed.
Act as a liaison between families and service providers (e.g., schools, behavioral health agencies, consultants) to ensure coordinated, adequate care and identify gaps in services.
