CVHS is seeking a full-time Behavioral Health Care Coordinator with a Bachelor’s degree in Social Work or related field to provide behavioral health care coordination, case management, and resource support services within an integrated primary care setting. Services will be provided to a diverse patient population with complex behavioral health and social needs.
The Behavioral Health Care Coordinator will assist with outgoing referrals for higher levels of care, conduct pre-behavioral health diagnostic screening visits, address social determinants of health, and assist patients with applications for disability and other social service resources. This role will also involve networking with community agencies, maintaining regional resource relationships, and collaborating closely with behavioral health clinicians and medical providers as part of a multidisciplinary care team.
While this position is primarily remote/work from home, there is an expectation to travel to assigned clinics as needed to meet with patients, network within the community, attend in-person meetings, and participate in occasional trainings. Candidates must reside in Virginia and preference will be given to applicants who are familiar with the Fredericksburg, Caroline, Westmoreland, and/or King William areas.
Candidates must be comfortable documenting all services in the electronic medical record in a timely manner and working in a fast-paced healthcare environment. Experience in behavioral health, healthcare, and/or case management preferred. Spanish-speaking preferred but not required. Competitive benefits available.
ESSENTIAL JOB FUNCTIONS:
Facilitate direct patient care provided in an integrated behavioral health care model for a diverse and underserved clinical patient population of all ages.
Accept and respond to inquiries about BHS in person, by phone, and electronically
Triage patient needs to determine if CVHS BHS can adequately assist patient
Complete appropriate, adequate, and timely documentation within the electronic medical record.
Provide mental health first aid for patients in crisis
Maintain guidance, reference documents for BH PSR’s on SharePoint
Train BH PSR’s in managing BHS schedules, referrals and insurance verification
Develop and maintain resource directory of services that are potentially useful for our patient population. In particular, knowledge of behavioral health services that we do not provide such as psychiatric care, psychological testing, and developmental services
Explore resource options as unique needs arise
Maintain data related to the entire behavioral health department as directed
Participate in CVHS organizational, and quality improvement meetings representing Behavioral Health.
Assist BH Director with creation of departmental reports
Promotion of integrated behavioral health services within and outside of CVHS.
Supports a team-based approach to patient-centered medical care.
KNOWLEDGE, SKILLS, AND ABILITIES:
Excellent customer service skills and abilities
Knowledge of, sensitivity to, and compassion for persons experiencing behavioral health crises
ability to triage BH patients (i.e., assess client BH clinical needs and determine if an appropriate fit for CVHS BHS, ability to assist patient in crisis)
Ability and willingness to assist patients having concrete needs comfort with technology for documentation, data management, and promotion to include use of the electronic medical record, data management software, and word processing software
Ability and willingness to work collaboratively with a multidisciplinary team
Willingness to participate in organizational quality improvement initiatives
Ability and willingness to promote BH services (e.g. represent BH at health fairs, and community meetings)

QualificationsEDUCATION, TRAINING AND EXPERIENCE:
Bachelor or Master Degree in social work or a human service field
Experience serving patients in a public behavioral health or medical setting
Mental Health first aid certification (can be obtained after employment)
