The Role

We are seeking a highly motivated and experienced or entry level Behaviour Support Practitioner to join our team in Swan Hill. This is an exciting time for us as this role is an extension to support the continued growth and reach of the business.

In this position you will be responsible for providing support to individuals with challenging behaviours, helping them to develop coping strategies and improve their quality of life. The role requires a compassionate and patient individual who is resilient and able to work collaboratively with clients, families, and other healthcare professionals.

Key Responsibilities:

• Conduct assessments to identify the root cause of challenging behaviours
• Develop individualised behaviour support plans (BSP), aiming to support and reduce the occurrence of behaviours of concern
• Assisting families and other support persons to implement BSPs
• Provide education and other supports to participants, their families, and caregivers on behaviour management techniques
• Monitor participant progress and adjust BSPs as necessary
• Maintain accurate and up-to-date records of client progress and interventions required to assess and adjust BSPs accordingly, including detailed report writing and other documents or recommendations as required
• Accurate and timely case noting is
• Participate in care team meetings and participate in case conferences as require
• Travelling up to an hour to attend face to face meetings with participants

This role will require you to travel up to an hour from Swan Hill to attend 1:1 meeting which can include in home or out in the community.

About you:

• Degree in psychology, social work, education, or a related field
• Previous demonstrated experience working with individuals with challenging behaviours
• Knowledge of behaviour management techniques and evidence-based interventions
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Compassionate and patient approach to working with clients
• Ability to travel including holding an Australian Drivers Licence with access to reliable car
