Position

  • Responsible for assessment, counseling and therapeutic interventions to individuals, families, and groups. The counselor collaborates with primary care providers, psychiatric staff and other multidisciplinary team members to support whole-person care and improve patient outcomes.

Education

  • Master’s Degree in Social Work or Counseling

Licenses and Certification

  • LCSW or LCPC in Illinois

Experience and Skills

  • Technical Experience: 2 years of experience in providing individual, family and/or group therapy required.
  • Strong interpersonal and communication skills
  • In-depth knowledge of community resources
  • Skill in diagnostic formulation (DSM-5)
  • Proficiency with EHR systems and documentation standards
  • Knowledge of trauma-informed and recovery-oriented care.

Physical Activities

  • Intermittent hand manipulation required
  • Intermittent lifting and carrying of 20 pounds

Role Specific Responsibilities

  • Conducts comprehensive biopsychosocial assessments, diagnostic evaluations, and treatment planning in accordance with clinical standards
  • Provides individual, family, and/or group therapy for patients experiencing behavioral, emotional, or mental health challenges
  • Collaborates with medical providers, psychiatrists, and advanced practice providers to coordinate care and ensure continuity of treatment.
  • Provide crisis intervention, risk assessments, and safety planning when indicated.
  • Support patients with referrals to a higher level of care or community resources when appropriate.
  • Maintain compliance with HIPPA, ethical guidelines, and all state/federal laws governing clinical practice.
  • Utilizes evidence-based practice standards to guide the evaluation of care, course of treatment, and appropriate use of organizational and patient-centered resources.
  • Develops and maintains collaborative relationships with organizations in the community that facilitate efficient and effective planning for continued care for the patient.
  • Collaborate with the patient, necessary staff, and post-discharge care providers to assure a safe and effective discharge plan.

Compensation (Commensurate with experience):

$57,595.20 - $89,273.60
