A Brief Overview
Provides care for the comfort, safety and personal needs of patients, acts as an advocate for the needs and rights of patients, provides empathic, active listening skills, engagement and support, case management and ensures a recovery oriented environment throughout the unit. In addition, provides indirect unit support needs related to supplies and equipment.

Essential Functions

    • Connects with new participants, on site or while the participant is incarcerated or hospitalized, to obtain referral information and necessary solutions
    • Teaches recovery and life skills using recovery tools.
    • Cultivates connectedness in the person’s life (self, others, the environment, and to meaning and purpose).
    • Assesses emergency situations, notifies supervisor of actual or potential challenges, exercises judgment in stressful conditions and responds calmly and quickly.
    • Completes clinical documentation as required.
    • Develops and maintains community resources for education, information and referrals appropriate to participant’s needs.
    • Understands and adheres to organization’s compliance standards as they appear in the Code of Conduct and Conflict of Interest Policies.
    • Performs other related duties as assigned.
    • Communicate with other team members on care plan as an active team member
    • Provide assistance to nurses with accompanying patients to the rest room when needed
    • Proactively utilizes de-escalation techniques to prevent harm.
    • Assist with linen changes and room/unit restocking
    • Assist with activities of daily living (ADLs)


Education Qualifications

  • High School Diploma Or GED Required

Experience Qualifications

  • Healthcare student or comparable work experience in a health care environment. Preferred

Skills and Abilities

  • Excellent communication and behavioral management skills that promote safety
  • Basic computer skills
  • Ability to problem solve
  • Ability to document per regulatory guidelines
  • Effective communication skills

Licenses and Certifications

  • BLS - Basic Life Support through the American Heart Association Upon Hire Required
  • Safety-Care Specialist within 60 Days Required
  • Minimum of 18 years of age Required
