In this pivotal role, you will provide direct care,overseeingdaily activities andmonitoringhealth, ensuring the safety and comfort of clients/patients. Your responsibilities will also includefacilitatinggroup activities, interpreting treatment plans, andmaintainingdocumentation, integral to the success of our program.

Key Responsibilities:
• Admit,supervise, monitor, transfer, and discharge clients/patients.
• Ensure monitoring of vital signs anddeterminethe need for escalation to medical intervention when critical symptomspresent
Facilitateclient/patient supervision checks per policy and/or procedure or whenindicatedby a safety/precautions plan.
Assistclients/patients with personal care as needed (e.g., grooming, bathing, dressing).
• Document reported behaviors and any areas of concern related to clients/patients.
• Encourage adherence to program expectations and communicate concerning behaviors to the treatment team promptly.
• Prepare for andfacilitategroup rehabilitative support, education, recovery skills, or recreation groups as scheduled and assigned.
• Provide client/patient transportation as needed to and from appointments.

Education, Experience, and/or Credential Qualifications:
• High School education OR equivalent and two (2) years of responsible employment.
• Experience in Human Services or related field preferred.
• Exceptions to the qualifications listed may be made by theappropriate ExecutiveTeam member.

Additional Qualifications:
• Current driver’s license, acceptable driving record, and current auto insurance.
• Ability toestablishrapport with client/patient populations whilemaintainingappropriate boundaries
• Successful completion of background check including criminal record, driving record, and abuse/neglect checks.
• Must be at least twenty-one (21) years of age tooperatea company-owned vehicle.

Physical Requirements:
• ADA Consideration - Sedentary work: Exerting up to 10 pounds of force occasionally and/or a negligible amount of forcefrequentlyto lift, carry, push, or pull, or otherwise move objects, including the human body.
• Sedentary work involves sitting most of the time but may involve walking or standing for brief periods of time.
