Department Overview

OHSU Family Medicine is part of the OHSU School of Medicine. OHSU Family Medicine is focused on family health. We provide preventive, primary and specialty care for you, your children, your spouse, your parents, and your grandparents. We provide all that family medicine has to offer, from maternity care through geriatrics. Family Medicine clinics are home to more than 120,000 patients every year, but despite the large number, our physicians, nurses, and other clinic staff know their patients and their families by name. Our patient care is among the nation's best, ranked #1 among the nation's medical school family medicine departments by U.S. News and World Report 2019, 2020, 2021 and 2023. Family Medicine strongly supports the comprehensive scope to provide the highest level of care for our community. The clinical work includes all ages of life including newborn to end of life. The Behavioral Health (BH) Supervisor position is a faculty position within the Department of Family Medicine (DFM) that oversees the behavioral health clinicians integrated across two Family Medicine clinics that are designated as Federally Qualified Health Centers (FQHC). The Supervisor is a member of the DFM BH Leadership Team including the Director of BH Education, Director of BH Clinical Operations and the BH Supervisor for non-FQHC sites. Each clinic team includes a Lead Behavioral Health Consultant who supports the local team and works under the direction of the BH Supervisor. This position requires some periodic travel as the

supervisor supports BH Team Members in clinical sites at the Richmond Clinic and the East Portland Clinic. This individual acts as a primary point person for clinic level leadership at these two clinics as it relates to the BH service line including but not limited to changes to service delivery, programming, and staffing (onboarding and recruiting).

This position provides PCBH clinical care as a core member of the integrated behavioral health care team at both Family Medicine FQHC locations. This position will provide a high degree of clinical and complex direct patient care as a Behavioral Health Consultant (BHC), it requires advanced-level generalist skills in delivering evidence-based interventions targeted to improve health and wellbeing for a large variety of patients within our Sports Medicine Team within the Department of Family Medicine.

• Director of Behavioral Health Clinical Operations.

Function/Duties of Position

Direct Patient Clinical Services and Documentation –

• Two days of direct patient care per week as an integrated behavioral health clinician as part of the clinic based Behavioral Health Team. This includes behavioral health interventions to family medicine patients with anxiety, depression, behavior change needs, eating disorders, insomnia, substance use disorders, and other mental health conditions. The primary care behavioral health model includes proactive patient identification (scrubbing schedules), brief interventions via same-day Warm Hand Offs, scheduled patient follow-ups, consultation to all care team members, assessments, and crisis intervention with safety planning.
• Participation in behavioral health case management as part of multi-disciplinary team to address complex patient needs. This includes both short- and long-term mental health care, and a high level of coordination with internal and community-based resources.
• Maintaining a professional presence for the clinic and actively engaging with teams and stakeholders.
• Documenting services appropriately in EHR in accordance with existing workflows to ensure continuity of care.
• Maintaining technology fluency in support of in-clinic and telehealth.
• Maintaining accessibility via department standard means (i.e., phone, pager, email)
• Maintaining appropriate professional boundaries as a part of the primary care team.

Behavioral Health Supervisor

• The breakdown for this portion of FTE includes scholarly and administrative time (0.15 FTE) with a dedicated amount of time for supervisory tasks (0.45 FTE)
• Supports clinical Behavioral Health Team staff across 2 FQHC clinics: Richmond (12 staff) and East Portland (4 staff)
• Works with DFM BH Leadership team to provide guidance and a framework to BH team members on how integrated behavioral health services are delivered within the context of community needs and clinic-level resources.
• Works with two assigned clinics and clinic leadership to ensure adherence to service excellence, ethical and compliance guidelines.
• Ensures that each clinic is appropriately staffed up to their allotted FTE. This includes ensuring the accuracy of position descriptions, screening applicants, organizing interviews and onboarding of new staff members. Advocates for increased staff when appropriate.
• Provides clinical support and direct supervision of all behavioral health staff across assigned Family Medicine Primary Care clinics (16 team members total). Collaborates with and supports clinic leadership teams to address complex personnel issues, training, onboarding, and delivery of appropriate clinical care.
• Provides clinical supervision to pre-licensed BHCs in pursuit of licensure. Identifies the formal and informal clinical consultation needs of BH team members assigned to you.
• Provides clinical consultation as needed and identified by BH team members to ensure appropriate clinical care. Works collaboratively with BH staff to identify supportive pathways to ensure that professional practice and growth needs are met.
• Provides recurring, regularly scheduled leadership, and support to BH Team Leads at 2 assigned FM clinics.
• Maintains capacity and knowledge base to allow cross-coverage with DFM BH Leadership team members to allow continuity of function in case of planned or unplanned absence.
• Education: Provide effective teaching to learners including medical residents, fellows, Master of Social Work Students and care team members regarding behavioral health issues, evidence-based interventions, primary care integration, and patient engagement/communication strategies.

Required Qualifications

• One of the following: PhD/PsyD or MSW/LCSW or LPC/LMFT (Oregon License required)
• Experience in behavioral health counseling, mental health assessment, substance use intervention, crisis intervention, case management.
• Demonstrated understanding of therapeutic interventions within the primary care setting, including CBT, motivation interviewing, solution and person focused interventions, ACES and the impact on overall health outcomes and health seeking behaviors.
• Demonstrated commitment to valuing diversity, equity and inclusion in the clinic and learning environment. Ability to contribute to the success of the organization through support of and seeking out diversified ideas and perspectives
• Demonstrated understanding of trauma informed and patient centered care medical home.
• Ability to work independently utilizing critical thinking, problem solving, and time management skills.
• Skill in developing and maintaining professional relationships and navigating differing ideas about care strategies.
• Capacity to manage technology supportive of in-person and telehealth including but not limited to electronic health record, virtual meeting platforms, and pager.
• Knowledge of the principles and practices of primary care behavioral health consultation, including assessment, diagnosis, treatment planning, counseling strategies, and their application to population-based care for preventive and chronic care issues.
• Demonstrated ability to practice with minimal supervision using sound clinical judgment

Preferred Qualifications

• Experience as a Behavioral Health Consultant in a Primary Care setting
• Experience in educational setting with supervision of graduate students
• Understanding of FQHC grants and reporting guidelines
• Interest and experience in program development, improvement, and assessment
• Language(s): Spanish, Vietnamese, Mandarin, Cantonese, Russian, Arabic, Korean, ASL

Additional Details

• Hours: Variable 40-hour M-F work week with possible weekends as needed
• Work location is both in-clinic and remote with majority of time in-clinic
• Frequent interruptions in an environment of frequent change and fluctuations
• May be exposed to body fluids, tissue, possible radiation exposure, and infectious processes
• Able to utilize office equipment including computer, smartphone, and pager in-clinic and remote
• Some travel may be required
• Ability to stand for four continuous hours a day
• Standing: On concrete, vinyl, or carpeting up to 4 hours per day and intermittently
• Sitting: Chair or stool continuous up to 8 hours per day and intermittently
• Changing Position: Intermittently, as needed
• Walking: Inermittently up to 8 hours per day on concrete, vinyl, and carpeting
• Bending: From the waist and knees, frequently throughout the day
• Reaching/Handling: Fine manipulation, repetitive, gross motor requiring full ROM in upper extremeties, including overhead reaching
• Twisting: All ROM required, intermittent periods, throughout the day
• Climbing: Staircase, intermittently throughout the day
