Job Description

The Behavioral Health Supervisor position is part of the Behavioral Health team at Ward 86, and is part of the clinical leadership at Ward 86. The Behavioral Health Supervisor is responsible for providing administrative and clinical supervision to Patient Navigators and other behavioral health staff who function as Patient Coordinators within Ward 86’s clinical programs. The Behavioral Health Supervisor may screen and assesses patients for service needs, psychosocial history and presentation, and develop and execute care plans. This position may be responsible for contract oversight, compliance, and contract deliverables. This position is part of a larger multidisciplinary HIV primary care team that includes medical doctors, nurses, pharmacists, social workers, patient navigators, and other clinical staff.

The central focus of this position is to plan and supervise the delivery of patient coordination services to patients living with HIV, individuals at risk of HIV acquisition, and patients living with high psychosocial complexity.

The Behavioral Health Supervisor will supervise the patient coordination services operations within assigned clinical programs, collaborating closely with the clinical leads of these programs. Working within a multidisciplinary management team, which includes Medical Providers, Registered Nurses, and other healthcare professionals, the Behavioral Health Supervisor will provide leadership and guidance on clinical matters, while also handling administrative responsibilities such as setting work schedules, resolving system issues, and managing complex cases and escalated clinical issues.

In addition to administrative and programmatic leadership functions, the Behavioral Health Supervisor may provide direct services to patients with complex psychosocial needs. This may include individuals experiencing homelessness or unstable housing, substance use, and complex mental illness. The Behavioral Health Supervisor will work collaboratively with internal care team members, as well as external case managers, care navigators, and other organizations and departments that provide supportive services to our patient population. The Behavioral Health Supervisor will have an opportunity to make a meaningful difference in the lives of the individuals we serve, through both rendering direct patient care and by ensuring that patient coordinators and allied staff are equipped with the knowledge, skills, and resources necessary to provide comprehensive care.

Direct Supervision

  • Responsible for recruitment, performance evaluation, progressive discipline and staff development of Patient Navigators and other direct reports. Develops a strong culture of teamwork to ensure the quality of daily operations, effective interdepartmental collaborations, and the timely achievement of objectives. Works with leadership to evaluate and set core competencies for direct employees.
  • Keeps abreast of and implements new or changing care standards, scope of practice, competency development and assessment, policies and procedures, and new programs.
  • Determines staffing needs and work plans for assigned areas of responsibility.
  • Manages and supervises clinical services rendered by direct reports at Ward 86, consistent with established standards and aligning with financial goals of organization and department.

  • Selects and hires qualified candidates for open positions and assures orientation to the clinical enterprise.
  • Conducts timely performance evaluations of direct reports based on organizational standards. Sets goals with employees being evaluated and supports employees in meeting career goals.
  • Consistently monitors and documents staff performance. Recognizes, coaches, counsels, and disciplines (as necessary) staff to achieve mutually agreed upon goals and organizational objectives.
  • Follows organization’s Human Resources policies and procedures and labor relations standards in interactions with staff.
  • Facilitates, and may oversee, resolution of human resources, labor relations, employee safety and risk management issues related to performance of direct reports.

Direct Patient Care

  • Performs psychosocial intakes and assessments for new and returning patients to Ward 86. Provides one-on-one counselling to patients for substance use, mental health, housing, and provides general psychosocial services.
  • Participates and leads direct deescalation efforts and crisis management as needed. Assesses patients for inter-personal or domestic violence, elder abuse and makes the appropriate referrals and police reports according to policies, and procedures.
  • Assists patients in accessing financial benefits and money management. Assesses and manages patients in matters pertaining to insurance coverage including ADAP enrollment.
  • Refers patients to community based case management agencies/organizations, substance use residential and outpatient treatment programs, mental health resources, psychiatry and psychotherapy.
  • Works with patients around transitional and permanent housing options through referrals to stabilization rooms and permanent housing options.
  • Responsible for coordinating and may directly provide coverage for subordinate staff as needed to ensure continuity of care across Ward 86 social services programs.
  • Provides general and specialized clinical services for patients as assigned.

Administrative Duties

  • Organizes behavioral health services delivery for assigned areas of responsiblity, including scheduling staff to ensure adequate coverage of assignments, responding to service interruptions, and developing programmatic responses to programmatic needs for behavioral health services.
  • Responsible for overall organization of behavioral health services for assigned areas of responsibility, including the development of new plans and programs to improve service delivery at Ward 86, and/or optimization of services across programs.

  • Develops and implements methods and processes related to behavioral health sevices at Ward 86. Participates in clinic wide programmatic development and leads development and implementation for direct employees.
  • As a part of the Ward 86 Leadership Team, may attend San Francisco Department of Public Health’s Primary Care Leadership Team meetings. Interfaces with Primary Care Director of Behavioral Health on matters related to behavioral health, such as care standards, quality improvement initiatives, and participates in meetings and consultations relating to this function. Attends other meetings, committees, and work groups related to functions as assigned.
  • Along with other members of the leadership team, the incumbent will lead responses to patient behavioral incidents on Ward 86. This involves setting behavioral expectations through policies, reviewing and responding to clinical reporting, and interfacing with patients and their representatives as needed.
  • Responsible for resolving patient complaints related to area of responsibility as assigned and in collaboration with other management team members.
  • Conducts outreach and educational activities in the community and at other agencies. Attends related contractual meetings. Participates in case conferences and meetingswith collateral providers to discuss clients’ care and service delivery, as needed.

MINIMUM REQUIREMENTS:

  • A master’s degree in behavioral health field (e.g., mental health, social welfare, counseling, psychology.
  • Demonstrated ability to effectively supervise a team and manage the complex workflow and multiple priorities involved with social work services.
  • Experience supervising case management staff.
  • Strong knowledge of behavioral health services, and experience with case management, utilization review, discharge planning, home care, and / or managed care.
  • Understanding of psychosocial issues facing the LGBTQ and HIV-affected communities.
  • Experience working within Harm Reduction treatment models, utilizing Motivational Interviewing interventions.
  • Strong knowledge of human resources management policies, with the ability to train, monitor, evaluate, and document human resources matters and performance, and to participate in decision-making on human resources matters.
  • Experience with contract compliance and monitoring.
  • Strong analytical and critical thinking skills, with the ability to quickly analyze problems, determine appropriate level of intervention, and develop and apply effective solutions.
  • Strong interpersonal and communication skills for effective collaborations with all levels of management and staff across clinical disciplines.
  • Knowledge of applicable business software and specialized applications and data management systems.
  • Demonstrated experience working with marginalized patient populations and culturally diverse populations including LGBTQ communities, and patients impacted by mental health, substance use and homelessness

PREFERRED:

  • Certification in psychiatric health specialty preferred.
