Schedule: Full-Time Days - Monday, Friday, Saturday (9:00 AM to 7:30 PM)

A Brief Overview
Serves as a contributing member on the Behavioral Health team; provides screening and assists with patient disposition plans that present with a health crisis. Helps Behavioral Health patients connect with resources that allow them to manage mental crisis as they arise; improve overall health and potentially avoid hospital encounters. Assists with posting discharge care and connecting patients to community resources and supports for individuals with complex circumstances.

Essential Functions

  • Provides brief interventions and psychoeducation to support patient wellness and recovery.
  • Works in a highly collaboratively manner with therapy clients' assigned case managers, and other service providers, within and outside facility.
  • Assists in developing measurable individualized therapy goals and objectives.
  • Serves as the primary contact for all payors regarding utilization review and management issues.
  • Delivers support of clinical integration needs of other clinical services, and collaboration with medical director leadership.
  • Maintains a patient first philosophy and engages in service recovery when necessary.
  • Participates in clinical huddles when working with medical & behavioral health providers to review the appointment schedule for the day and determine which patients will need visits.
  • Ensures that documentation compliance requirements are met consistently.


Education Qualifications

  • Bachelor's Degree from an accredited college or university in Social Work or related field Required

Experience Qualifications

  • Experience working in a clinic or hospital setting Preferred

Licenses and Certifications

  • BLS - Basic Life Support through the American Heart Association Upon Hire Required and
  • Safety-Care Specialist within 60 Days Required
